Diabetes Care Center
As the Diabetes Care Center’s Medical Director, Dr. Pazooki and the staff at the Diabetes Care Center (DCC) provide Diabetes self Management Training (DSMT) and Education to individuals and groups of all ages. Clientele are guided through learning survival skills, basic care and intensive diabetes self management techniques. Those receiving services at the DCC receive guidance in the following areas:
- How to understand diabetes mellitus: its types, causes and effects, and the goals of treatment
- Management skills for all types of diabetes and related issues
- Planning and creating lifestyle changes: healthy eating habits, meal planning, physical activity, monitoring health status, taking medications, problem solving, healthy coping, risk reduction, weight management and prevention of hypoglycemia (low blood sugar)
- Self monitoring of Blood Glucose (SMBG)
- Medications and diabetes care
- Carbohydrate Counting
- Intensified insulin treatment
- Insulin pump therapy, initiation & titration
- Avoidance of complications related to diabetes
The Care Center maintains recognition with the American Diabetes ssociation as a diabetes education entity.
